How do spreadsheet tabs become Databricks tables?

That matches how people use spreadsheets, where one file holds several related but separate things. With headers enabled, row one supplies the column names. Without headers the columns arrive lettered, which works but is rarely what you want to query against later.

Will long text in a cell get cut off?

No. Databricks string columns are created as unbounded STRING, and even a source column with a declared limit maps to STRING, so there is no truncation step on the way in. That is a genuine difference from warehouses with a fixed maximum string width, and it matters if your sheet holds free text notes or pasted descriptions that would otherwise be clipped and flagged.

How are spreadsheet column types decided?

Conservatively. Values default to strings unless the column carries a data validation rule marking it as a number, date or boolean, in which case that type is used. Adding validation rules on the sheet is the reliable way to get proper typing. Doing it before the first sync saves effort, because changing a column type later archives the old column under a name carrying its previous type and adds a new one beside it.

What breaks if a column gets inserted?

Your mappings, quietly. Fields are tracked by position, as column A, column B and so on, rather than by the header text above them. Insert a column in the middle and every mapping after it now reads the wrong source, while the sync carries on writing plausible looking values under the right names. Nothing errors. After any structural edit to the sheet, refresh the schema and check the mappings before trusting the next run.
